7 La vita 25 GIUGNO 1933 Nasce a PESCARA da Vito Rastelli, giornalista e sindacalista, e Luisa Bianchi, maestra elementare. Vive poi a REGGIO EMILIA, ROMA, SONDRIO. Nel 1945 si trasferisce a PARMA con i genitori e la sorella minore Rosangela. Frequenta il Liceo Classico Romagnosi. In questi anni incontra Padre Molin Mosè Pradel dei gesuiti della chiesa di San Rocco Si iscrive alla facoltà di Medicina e Chirurgia nell'università degli Studi di Parma e inizia la sua vita in ospedale come interno presso l'istituto di Anatomia normale, poi presso la Clinica di Patologia Generale. Nel 1957 si laurea con 110 e Lode con la tesi Changes in the ATPase activity of the myocardium during deep hypothermia, premiata come migliore dell anno (premio Lepetit ). 7 SETTEMBRE 1961 Parte per l'america grazie a una borsa di studio NATO a bordo della nave Queen Federica. Completata la scolarship, Gian rimane alla MAYO CLINIC (ROCHESTER, MINNESOTA) come Research Assistant ( ) e poi come Research Associate ( ). Nonostante la malattia nel 1968 diviene responsabile della ricerca in cardiochirurgia. Sono per lui anni di lavoro instancabile diviso tra clinica, ricerca e scrittura di lavori scientifici. Il 12 AGOSTO 1964, durante uno dei suoi periodici viaggi in Italia, sposa nell abbazia di Chiaravalle della Colomba (Piacenza) Anna Anghileri, conosciuta a Bormio sulle piste da sci due anni prima. Nel 1966 nasce la figlia Antonella Luisa Inizia subito a lavorare come assistente volontario presso la Clinica Chirurgica I di Parma sotto il professor Antonio Bobbio. In questi anni è secondo medico ufficioso di Polesine Parmense. È un periodo di studio e di intense pubblicazioni scientifiche che gli varranno la borsa di studio NATO FEBBRAIO 1970 Il 29 GENNAIO 1970 non si presenta al meeting in cui avrebbe dovuto illustrare alla sua equipe il III metodo Rastelli. Gian viene ricoverato. Muore al Methodist Hospital di Rochester, sede della Mayo Clinic. Di ritorno dal viaggio di nozze gli viene diagnosticato un linfogranuloma maligno o linfoma di Hodgkin.

18 L esame obiettivo 18 Nelle visite che faceva ai piccoli pazienti dedicava molto tempo alla raccolta dell anamnesi, alla storia di quel piccolo paziente e a tutto quello che i genitori gli spiegavano. Il tempo che dedicava all auscultazione del cuore era il più lungo, quasi volesse scavare il percorso del sangue nel cuore. Non c era l ecografia, ma la sua logica e la sua speculazione erano preminenti. Spiegava poi ai genitori su un foglio bianco che cosa aveva il bambino e come si sarebbe proceduto chirurgicamente. Dava sicurezza e speranza alla ricerca della condivisione terapeutica.

Ogni mezz ora usciva per darmi fiducia: «Siamo a questo punto, signora, tutto procede bene». E, alla fine, uscì con un sorriso di vittoria per noi e mi annunciò: «Tutto fatto, meglio di così non poteva andare».

Una specie di tacito patto si instaurò tra noi, di non parlare di questo male, a meno che non vi fosse una urgentissima necessità di farlo. Il dottor Rastelli era troppo intelligente per non rendersi conto che il suo male era fatale. Eppure lavorava con felicità, con vigore e con straordinari risultati, senza il minimo pensiero, in apparenza, che la sua vita avrebbe avuto una fine prematura. La serenità e fiducia con cui il dottor Rastelli affrontò la vita e la morte, letteralmente, la più grande delle molte cose che egli mi ha insegnato»

26 A JOURNEY! PROJECT ON DR RASTELLI We are medical students from the University of Bologna (Italy) who came across the story of the surgeon Giancarlo Rastelli while studying for our exams. We were impressed by his humanity and decided to prepare a small exhibit in order to let our teachers and colleagues know about this man. We eventually wrote a biography about him. During the last year we spent a lot of time going around Italy in schools, hospitals and meetings in order to tell people what we discovered about him. Giancarlo Rastelli (1933, Parma 1970, Rochester) was an Italian doctor who made important discoveries in pediatric cardiac surgery. Since his youth he was in love with life : he was fond of reading, he loved classical music, he used to go skiing on the Dolomites with his friends, fishing on the Po River... But most of all he was fond of medicine, as some of his friends told us. Despite the brevity of his life he developed the Rastelli Classification for the Complete Atrioventricular Canal defect (CAVC) and invented the Rastelli procedure for treating the Truncus Arteriosus and the Transposition of the Great Arteries with Ventricular Septal Defect and Pulmonary Stenosis. His most productive years were spent in Minnesota at the Mayo Clinic. He got there in 1961 thanks to a NATO scholarship. He died in 1970 due to a Hodgkin's lymphoma. At first we were struck by his approach to surgery. He wrote John Kirklin [who was his master], while being the surgeon who put patches into his patients hearts, much more importantly knows everything about the physiology of the heart, about catheterization, angiocardiography, pathological anatomy, embryology. And it is just that that makes him a great surgeon. At the same time in his work he paid great attention to his patients and their humanity. In a letter to a friend he wrote: the first kindness (charity) that the patient must receive from a doctor is the kindness of science. It is the kindness of being cured in a way that works. Without this, it is useless to speak of any other kindness. He also wrote Knowing without knowing how to love is nothing. It s less than nothing. As a matter of fact, he helped several Italian families to bring their children to the Mayo Clinic in order to be adequately treated for their congenital hearts defects. Doctor Rastelli managed to collect enough money to support them in bearing travel expenses and he did everything possible to make them feel at home. He continued to take care of them in this way even during his last days of life. In fact, he died of lymphoma at a young age. As John W. Kirklin, MD wrote: "Perhaps the more remarkable aspect of the life of this man was the reaction to his fatal illness. [ ] Dr. Rastelli was too intelligent not to realize that his illness was fatal. Yet he was working with happiness, with vigor and extraordinary results without the least thought, apparently, that his life would have had a premature end. The serenity and confidence with which Dr. Rastelli met life and death, literally, is the largest of the many things he taught me. He considered his profession as a vocation. Questions for the Interview 1) Giancarlo Rastelli had been a medical student and a scientist, a husband and a father, and (at the end of his life) he became also a patient. For him, these different aspects were not separate fragments, but part of the same life. One thing that strikes us looking at the life of Dr Rastelli is a unity, his passion for life as a whole.. a. Do you feel unity in life is important? What does it bring unity to your life? Is there something or someone that keeps unity into your life? 2) Dr Rastelli spent the last days of his life giving himself to what and to whom he loved until the very end. a. Would I continue to do the same things that I am doing now even if they were the last days of my life? What gives meaning to my life? What (or whom) am I giving my life for/to? 3) Dr Rastelli became a great doctor and scientist in the nourishing environment of Mayo Clinic. At the basis of the Mayo Culture is the needs of the patient come first. a. How does the Mayo culture create and facilitate this way of caring for the patient? How do you define patients need? Is it different from patients wish? 4) Dr Rastelli was a doctor, taking care of the sick and then a patient, at the end of his life. a. What is special in medical care, vulnerability and illness that allow the basic questions of life to surface? Do you ever find/see any meaning in sufferance? Do you have any concrete examples of patients or in your personal life? 5) Giancarlo Rastelli grew up in an environment of collaboration and friendship. a. What is friendship for you? Is it different from teamwork? Is it possible to have friends in the work environment?
