2 ABSTRACT Increased intraocular pressure (IOP) is the leading cause and the hallmark of Glaucoma, an asymptomatic and irreversible degenerative disease that, undetected or untreated, progresses to blindness due to optic nerve atrophy. Glaucoma is the second cause of blindness and visual impairment worldwide. Peripheral visual field is first affected, often unnoticed by the patient, progressing towards the loss of the central visual field and ultimately towards blindness. Continuous monitoring of the IOP is of major importance both for a better management of Glaucoma treatment, and for a more efficient and earlier screening of Glaucoma patients amongst risk groups. Currently, the only way to get data on the dynamic behavior of the IOP is to repeat punctual measurements several times a day. No available technology can perform uninterrupted 24-hour IOP measurements during sleep or during daily activities. Consequently, the ophthalmologist does not have the tool that permits widespread, cost-effective, and precise recording of the IOP for the full 24- hour period, in the case of Glaucoma patients or for Glaucoma screening. As a result, the disease remains under-diagnosed and many Glaucoma patients are kept on insufficient or inappropriate medication. This research work was focused on the development, characterization and testing of a novel minimal invasive microsystem for the monitoring of IOP, based on a new measurement principle. The research work presented in this thesis combines microtechnology, microelectronic and biomaterials. The method of indirect continuous monitoring of IOP presented here is based on the detection of spherical deformations of the eyeball (changes in cornea curvature) due to IOP. We developed and patented a soft contact lens with a microfabricated thin film strain gage sensor embedded, which is capable of measuring cornea deformations due to IOP variations the Contact Lens Sensor (CLS). To power the sensor and retrieve data from the contact lens wirelessly, telemetry based on absorption modulation has been investigated and integrated in the CLS. The fabrication technology is based on a well known polyimide-based technology that has been widely used to fabricate implantable microelectronics, but two critical fabrication steps have been addressed and improved: metal adhesion on polyimide and structures release from wafer after fabrication. As the application is related to the medical field, great importance has been given in the choice of biocompatibility materials. The developed fabrication technology in combination with telemetry by absorption modulation, the flip-chip-on-flex assembly technology and the silicone biocompatible encapsulation and insulation technique open up new possibilities for the development of implantable microsystems for human biometric parameters monitoring and animal long term monitoring and testing, where small wireless sensors and no battery are needed. i

3 Prototypes of the CLS have been tested on pig-enucleated eyes, under simplified physiological conditions in order to verify our novel measurement principle for the IOP monitoring. Moreover the first wired prototype has also been tested on humans to study and investigate all issues related to safety, comfort, stability and reliability of the CLS. The results presented show that the device is sufficiently sensitive to measure a signal equivalent to the human ocular pulsation (OP) and follows well the IOP variations in a reproducible way, in static and dynamic mode, validating therefore the measurement principle and theoretical calculations. Next step will be extensive clinical trials to validate the systems and its reproducibility on humans. The wireless CLS would allow for the first time minimally invasive continuous IOP monitoring over periods up to 24 hours, regardless of patient position and activities. It would permits 24-hour continuous IOP measurements performed during sleep and various independent activities without the involvement of the patient, thus opening up new diagnostic and therapeutic methods to manage Glaucoma. 4 VERSIONE BREVE Una pressione intraoculare elevata (PIO) est la causa et il segno distintivo del glaucoma, una malattia asintomatica e irreversibile che, se non diagnosticata e trattata, porta alla cecità, in cosegueza alla degenerazione del nervo ottico. Il glaucoma è la seconda causa di cecità e perdita di campo visivo nel mondo. La perdita interessa in primo luogo la visione periferica, sovente non notata dai pazienti, e progredisce poi con la perdita di visione anche nella regione centrale, fino ad arrivare alla cecità. Il monitoraggio continuo della PIO è estremamente importante per una migliore gestione e trattamento della malattia e per un depistaggio più efficiente e precoce delle persone a rischio. Attualmente l unico modo di avere un indicazione del comportamento dinamico della PIO è quello di ripetere più misure puntuali lungo l arco della giornata. Nessuna tecnologia attuale permette una monitoraggio continuo della PIO. L oftalmologo non ha quindi lo strumento per effettuare delle misure di PIO sulle 24 ore e di consegueza la malattia non è sempre correttamente diagnosticata e molti pazienti affetti da glaucoma ricevono cure insufficienti o inappropriate. Questo lavoro di ricerca è focalizzato nello sviluppo, la caratterizzazione e il test di un innovativo microsistema per il monitoraggio continuo della PIO. Il sistema è minimalmente invasivo et si basa su un nuovo concetto di misura attraverso la detezione della deformazione del globo oculare che è legata alla variazione di pressione. Il sistema sviluppato è stato brevettato e l elemento chiave è una lente a contatto con un sensore di stress integrato Contact Lens Sensor (CLS). Per alimentare il microsistema et transferire le misurazioni della PIO senza fili, un sistema telemetrico basato sulla modulazione per assorbimento è stato studiato e integrato anch esso nella CLS. La tecnologia di fabricazione è basata su una tecnologia nota che è utilizzata anche per realizzare elettrodi flessibili impiantabili nel corpo umano, ma due tappe fondamentali della fabbricazione che presentavano importanti lacune sono state studiate e migliorate: l adesione delle piste metalliche sul substrato flessibile in polyimide e il distacco delle strutture dal supporto di fabbricazione. Siccome l applicazione è nel campo medico, particolare importanza è stata data alla scelta di materiali biocompatibili. La tecnologia di fabricazione sviluppata, in combinazione con il sistema di telemetria, la tecnica di assemblaggio flip-chip e la tecnica di incapsulazione in silicone, aprono nuove possibilità per lo sviluppo di microsistemi impiantabili per il monitoraggio di parametri biomedici, dove è necessario avere dei sensori miniaturizzati che funzionano senza filo e senza batteria. I prototipi realizzati della CLS sono stati testati su occhi di maiale enucleati per verificare il principio di misura della PIO. In più, il primo prototipo, che aveva dei fili di collegamento al posto del sistema telemetrico, è stato testato anche in clinica per mettere in evidenza tutti i potenziali problemi legati alla sicurezza, al confort, alla stabilità e alla riproducibilità della CLS in condizioni reali di utilizzazione. I risultati presentati mostrano che il sistema è adeguatamente sensibile per misurare un iii

5 segnale equivalente alla pulsazione oculare et segue le variazioni della PIO in maniera riproducibile, sia in modo statico che in modo dinamico. Il passo successivo sarà uno studio clinico della esteso della versione finale della CLS senza filo per validare il sistema, la sua riproducibilità e la sua stabilita sui pazienti. La CLS permette per la prima volta un monitoraggio continuo e minimalmente invasivo della PIO per un periodo fino a 24 ore, indipendentemente dalla posizione dall attività e dalla posizione del paziente e aprendo quindi nuove e importanti possibilità diagnostiche e terapeutiche per gestire e curare il glaucoma.
