Principal Investigator del Protocollo di Studio:A Phase 1/2 Study to Evaluate the Safety, Tolerability, and Pharmacokinetics of MEDI4736 in Subjects With Advanced Solid Tumors ~ OBIETTIVI RAGGIUNTI Nell'ambito della sua attività il dr Ascierto ha raggiunto i seguenti obiettivi. 11

12 Ricerca di base a) ruolo della 9p21 durante lo sviluppo del MM (prima della progressione neoplastica); b) una nuova regione di perdita di eterozigosi ( che potrebbe indicare la presenza di un gene oncosoppressore candidato) nei tumori primitivi. e) La valutazione su linee cellulari di melanoma di nuovi farmaci biologici (inibitori di BRAF, MEK, PI3K, AKT, Brivanib, etc): i. ha dimostrato che gli inibitori di BRAF (dabrafenib) sono attivi non solo sulla mutazioni V600E/K di BRAF, ma anche su quelle minori (V600D/R). ii. il test dell 'ABT-888, un nuovo inibitore delle PARP, ha evidenziato su tali linee una differente risposta in relazione all'identificazione della mutazione del gene BRAF.. Diagnosi precoce Il dr Ascierto si è affermato in campo internazionale per la qualità del servizio fornito, per la rilevanza scientifica e per l'elevato numero dei casi che afferiscono alla struttura da lui coordinata. Inoltre, in collaborazione con il Prof. Alistair Cochran delpucla di Los Angeles, è stato il proponente delle linee guida per il trattamento delle lesioni pigmentate,della cute; tale lavoro cooperativo ( che vede la partecipazione di importanti esperti del settore appartenenti all'istituto di Oncologia Europeo di Milano, all'istituto Dennopatico dell'immacolata di Roma, al Centro di Riferimento Oncologico di Ravenna) è stato pubblicato nel 2003 e rappresenta, al momento, lo standard di riferimento internazionale per il management dì tali lesioni. Il dr Ascierto ha offerto la consulenza informativa e diagnostica a popolazioni della Regione Campania (ed anche extraregionale) attraverso un'unità operativa mobile messa a disposjzione dall'house Hospital, ed indirizzato i pazienti sottoposti a screening al miglior trattamento medico e chirurgico disponibile offrendo dei canali di prenotazione automatici per il management di questi pazienti presso l'istituto Tumori di Napoli.. Attraverso la Fondazione Melanoma Onlus (di cui è il Presidente), si è reso promotore della realizzazione del calendario della prevenzione nonché delle pillole della salute con personaggi di cartoni animati dedicati a messaggi chiave per la popolazione. Entrambe queste iniziative s0no state realizzate in collaborazione con la Scuola napoletana di Comix e con i comici del programma televisivo "Made in Sud''. Ricerca applicata alla clinica: biomarcatori L'utilizzo di tecniche di biologia molecolare è stata rivolta alla valutazione dell'estensione della malattia. Il dr Asci.erto ha collaborato allo sviluppato di un protocollo basato sulla RT PCR (Reverse Transcriptase-Polymerase Chain Reactìon), che permette di rivelare la presenza di cellule metastatiche di melanoma nel sangue periferico (identificando RNA messaggeri specifici delle cellule melanocitiche) utilizzando marcatori molecolari multipli (RT-PCR di tipo "multiplex", con impiego di quattro marcatori: tyrosinase; p97, MUC18 e MelanA/MARTl). Tuttavia, nonostante risultati preliminari avevano evidenziato una correlazione tra la presenza di antigeni circolanti melanoma~specifici e la progressione tumorale (follow-up mediano di 13 mesi), l'analisi multivariata effettuata sullo stesso campione di pazienti (follow-up mediano di 42 mesi) non ha confermato tale correlazione dimostrando che fattori come l'età e, soprattutto, lo stadio rappresentano variabili più forti nel valore prognostico predittivo di ricaduta. Tali dati sono stati pubblicati sul numero di marzo 2003 di J Clin. Onco!. e, visto il notevole impatto sulla pratica clinica, sono stati pubblicati nelle News Online sul sito dell 'American Society for Clinica! Oncology. Inoltre, in collaborazione con l'istituto di Genetica Molecolare del C.N.R. di Alghero, ha contribuito allo sviluppo di nuovi protocolli di RT-PCR per l'identificazione di 12

13 micrometastasi occulte nei linfonodi regionali di pazienti con melanoma. La presenza di cellule occulte in tessuti inclusi in paraffina da linfonodi regionali negativi è stata dimostrata statisticamente correlata sia allo stadio di malattia che, soprattutto, alla presenza di recidive. In particolare, l'analìsi statistica effettuata su di un can1pione di 75 pazienti ha rivelato il valore prognostico della positività a tali marcatori (tirosinasi e MelanA/MARTl ); il gruppo di pazienti con positività ad entrambi i marcatori ha mostrato un tempo di progressione della malattia (follow-up 36 mesì) inferiore agli altri gruppi (:Sl marcatore). Tale risultato va confermato su una più ampia serie di pazienti. L'elemento innovativo dello studio è la possibilità di effettuare l'rt-pcr su cdna estratto da tessuto incluso in paraffina e quindi consentire un'analisi retrospettiva. Nel 2011, è stato il responsabile per i1 trattamento di circa 150 pazienti con l'anticorpo monoclonale Ipilimumab. Una delle criticità della terapia con tale anticorpo è stata la possibilità di individuare i pazienti responders da quelli non responders ma ad oggi non sono stati ancora individuati marcatori in grado di predire tale risposta. Per tale motivo è stato avviato lo studio di valutazione delle cellule T regolatorie (Treg) circolanti e la loro con-elazione con altri parametri ematochimici quali la PCR (proteina C reattiva) e l'ldh. I dati ottenuti evidenziano due elementi impo11anti: l'aumento della percentuale di cellule Treg circolanti si correla con una non risposta al trattamento; una diminuzione dei valori di LDH e di proteina C Reattiva è riscontrata nei pazienti responders. Infine, sempre al fine della possibile individuazione di biotnarcatori predittivi di risposta, ha avviato lo studio di valutazione delrinfiltrato immunitario nei campioni istologici dei pazienti affetti da melanoma (Immunoscore). Egli, infatti, fa parte dello Steering Committe della World Wide Task Force per la determinazione dell'immunoscore nel Cancro. Terapia adiuvante Il dr Ascierto si è affermato in campo internazionale per l 'affidabilìtà dei dati rìportati e le capità scientifico-assistenziali. Nel corso degli ultimi anni ha pubblicato alcune review sulla terapia adiuvante del melanoma che hanno suscitato l'interesse internazionale ed lo hanno posizionato al centro del dibattito internazionale nel can1po (key opinion leader). Inoltre, ha partecipato ai più importanti trial internazionali con i nuovi farmaci: ipilimumab, vaccinazione con MAGE-A3, vemurafenib, nivolumab, pembrolizumab, Terapie per le forme avanzate Si è reso protagonista dello studio di nuovi schemi di polichemioterapia (Temozolamide + CDDP) nonché della sperimentazione di nuovi farmaci. Terapie innovative Nel corso del 2002 ha attivato, in collaborazione con l'università del Kentucky e con l'md Anderson di Houston, Texas, USA, un trattamento sperimentale di fase I con l'inibitore dell'arginina Deaminasi (ADI-PEG 20) per i pazienti con melanoma metastatico pluritrattato; tale terapia prevede semplici iniezioni intramuscolari settiman.ali. I risultati preliminari depongono per una safety, notevole tollerabilità ed attività antineoplastica del fannaco per cui non è difficile ipotizzare, per il futuro, uno stravolgimento della terapia del melanoma avanzato. Ha partecipato attivamente allo sviluppo di nuove molecole come ipilimwnab, vemurafenib, dabrafenib, nivolumab. MEKl 62, vismodegib, pembrolizumab, etc. La pubblicazione dei dati scientifici attinenti tali studi e l'invito in diversi congressi nazionali ed internazionali, lo confermano come key opinion leader per il melanoma ed i tumori cutanei in generale. È membro di numerosi advisory board internazionali. 13
