PRINCIPALI PROBLEMATICHE GASTROENTEROLOGICHE NEL PAZIENTE CON MIELOLESIONE Patologia Peptica e da Reflusso Gastro-Esofageo (esofagite, gastrite da stress, otite, inalazione) Alterazioni del circolo entero-epatico degli acidi biliari (formazione di calcoli in colecisti, fango biliare, colestasi enzimatica, screzio pancreatitico, deficit digestivi) Contaminazione batterica del Tenue (malassorbimento, SIRS, alterata permeabilità epiteliale, ileo dinamico, diarrea secretiva e/o osmotica) Malnutrizione (alterazioni della composizione corporea, carenze di oligoelementi, immunodeficienza, comparsa di LDD) Disfunzione del colon (ritenzione fecale, incontinenza, bloating, patologia proctologica, manifestazioni disautonomiche, infezioni alle LUTs)

This indicates further the existence of a limited number of well-balanced host microbial symbiotic states that might respond differently to diet and drug intake. Malnutrizione nel mieloleso in fase acuta Si stima che una perdita di peso intorno al 10% sia comunque presente nei pazienti con SCI, nonostante un adeguato (o in eccesso) apporto calorico Il bilancio azotato negativo e la marcata perdita di 3-metilistidina sono fasi obbligate, secondarie alla atrofia muscolare e alla ridotta REE, a loro volta conseguenza della denervazione delle fibre muscolari Non esistono studi controllati che forniscano evidenze sul ruolo della NA nei pazienti con SCI in fase acuta. I dati sono traslati dagli studi nei pazienti con TCE Utilizzare primariamente la via enterale rispetto alla parenterale, garantendo integrità funzionale intestinale, minore rischio infettivo, ridotte complicanze e minori costi rispetto alla via iniettiva Dove coesistano danni intestinali, ileo meccanico o dinamico prolungati, e la via di somministrazione parenterale sia obbligata, si consiglia appena possibile il passaggio alla NE integrativa o totale L apporto calorico totale è valutato intorno alle 28 KCal/Kg/die, la quota proteica varia in un range compreso tra 1 e 2 g/kg/die. La definizione dell apporto lipidico non è certa: alcuni studi di classe II suggeriscono che il contenimento della quota lipida ridurrebbe morbilità e migliorerebbe l outcome
FABBISOGNO ENERGETICO Il dispendio energetico globale di un paziente (Total Energy Expenditure o TEE) comprende varie componenti: BEE: (Basal Energy Expenditure), rappresenta il dispendio energetico basale, viene misurato con la calorimetria indiretta oppure con la formula di Harris-Benedict diversificata secondo età e genere: Uomini: 66.5 + [13.75 x peso attuale in Kg] + [ 5 x altezza in cm] [ 6.75 x età in anni] Donne: 65.5 + [9.75 x peso attuale in Kg] + [ 1.85 x altezza in cm] [ 4.67 x età in anni] Bambini: 22.1 + [31.05 x peso attuale in Kg] + [ 1.16 x altezza in cm]. REE: (Resting Energy Expenditure), rappresenta il Dispendio Energetico a riposo, equivalente alla spesa energetica dell organismo per mantenere le funzioni vitali ( circolazione sanguigna, attività respiratoria, funzioni del sistema nervoso, attività ghiandolare e mantenimento della temperatura corporea). Il suo valore è massimo nell organismo in crescita (circa 53 Kcal/Kg/die ad 1 anno) e decresce con l aumentare dell età (31 Kcal/Kg/die). Presenta inoltre valori più bassi nella donna rispetto all uomo, ed aumenta del 13% ogni aumento pari ad 1 C di temperatura corporea maggiore di 37. Rappresenta il 60-70% del TEE e viene calcolato moltiplicando il valore della BEE con di fattori di correzione definiti Fattori di Stress e Fattori di attività, scegliendo un solo fattore di correzione per categoria.
