ABSTRACT (English) Feedback and feedforward mechanisms for the control of lower limb posture following anterior cruciate ligament rupture and reconstruction When unpredictable and predictable perturbations occur, the central nervous system controls posture throughout compensatory and anticipatory postural adjustments (CPAs and APAs), which are regulated by means of feedback and feedforward mechanisms, respectively. The ability to minimize postural disturbances when unpredictable and predictable perturbations occur plays an important role in preventing injuries of the lower limb. It has been shown that the rupture of the anterior cruciate ligament (ACL) occurs when a sudden postural perturbation appears during voluntary movement of the lower limb. In addition, after ACL rupture and reconstructions patients have shown impairments in the ability to adequately respond to sudden unpredictable and predictable whole-body perturbations. These condition may lead to a higher risk for re-injury. However, whole-body perturbations used by previous studies do not allow to isolate knee joint function, which in these circumstances is controlled and regulated in accordance with the rest of the body, and knee joint functional deficits, which in the same circumstances may be blunted or concealed by compensations implemented by other body segments. The objective of this research was to design a novel perturbation task specific for the knee joint to assess CPAs and APAs by means of unpredictable and predictable perturbations following ACL rupture and reconstruction with the aim to identify and quantify the deficit in postural control mechanisms. Delayed compensatory responses to unpredictable perturbation and early anticipatory responses to predictable perturbations were found before ACL reconstruction showing that neuromuscular impairments and safety strategies associated to ligament rupture cause abnormalities in both mechanisms. After surgical reconstruction of the ACL a gradual recover of normal APAs is observed in the first six months post-surgery showing that safety strategies disappear and normal APAs are restored. On the contrary, delayed responses to unpredictable perturbations were still observed six months after surgery thus showing that CPAs are still impaired at the time of return to unrestricted sport activities. Returning to sport with the inability to adequately respond to sudden unpredictable perturbations increases the risk of re-injury.
The results of the present research shows that it is possible to identify impairments in postural control mechanisms by means of the novel task proposed. Future studies should clarify if it is possible to reverse abnormalities in postural control mechanisms designing appropriate rehabilitation programs addressing motor control theories.
ABSTRACT (Italiano) Meccanismi a feedback e feedforward per il controllo della postura nell arto inferiore in seguito a rottura e ricostruzione del legamento crociato anteriore All arrivo di perturbazioni imprevedibili o prevedibili, il sistema nervoso centrale controlla la postura per mezzo di aggiustamenti posturali compensatori e anticipatori (APC e APA), i quali sono regolati da meccanismi a feedback e feedforward, rispettivamente. L abilità di minimizzare gli effetti destabilizzanti di perturbazioni posturali imprevedibili e prevedibili ha un ruolo fondamentale nella prevenzione degli infortuni all arto inferiore. La rottura del legamento crociato anteriore (LCA) avviene spesso quando una perturbazione posturale improvvisa si presenta durante il movimento volontario dell arto inferiore. I pazienti con una ricostruzione del LCA hanno inoltre mostrato deficit nella capacità di contrastare adeguatamente perturbazioni posturali applicate all intero corpo. Questo molto spesso rappresenta una delle maggiori cause di re-infortunio. Tuttavia, le perturbazioni riguardanti l intero corpo utilizzate dagli studi presenti attualmente in letteratura non permettono di studiare in maniera isolata la funzione dell arto inferiore, che in queste circostanze è controllata e regolata concordemente con il resto del corpo, ed i deficit nella funzione dell arto inferiore, che nelle stesse circostanze potrebbero essere mitigati o mascherati dalle compensazioni generate dagli altri segmenti corporei. L obiettivo di questa ricerca è stato quello di proporre una nuova condizione sperimentale caratterizzata da perturbazioni posturali specifiche per l articolazione del ginocchio finalizzate a valutare gli APC e gli APA per mezzo di perturbazioni imprevedibili e prevedibili in seguito alla rottura e ricostruzione del LCA con lo scopo di identificare e quantificare i deficit nei meccanismi di controllo della postura. Nei pazienti con rottura del LCA sono state osservate risposte ritardate a perturbazioni imprevedibili e risposte precoci a perturbazioni prevedibili, mostrando quindi che le alterazioni neuromuscolari e le strategie di protezione tipiche in caso di rottura del LCA hanno effetti negativi su entrambi i meccanismi. In seguito alla ricostruzione chirurgica del LCA è stato osservato un graduale recupero di normali APA nei primi sei mesi dopo l intervento mostrando che le strategie di sicurezza adottate spariscono ed una normale attività anticipatoria viene ricostituita. Al contrario, risposte ritardate a perturbazioni imprevedibili sono state osservate nei sei mesi successivi alla ricostruzione chirurgica del LCA, mostrando quindi che gli APC sono ancora alterati al tempo di
ritorno alle attività sportive senza restrizioni. Tornare allo sport con dei deficit nell abilità di rispondere adeguatamente a perturbazioni imprevedibili può aumentare il rischio di re-infortunio. I risultati di questa ricerca mostrano che è possibile identificare le alterazioni nei meccanismi di controllo della postura utilizzando la nuova tipologia di perturbazioni proposta. Studi futuri dovranno chiarire la misura in cui sia possibile eliminare, attraverso l allenamento, le anomalie nei meccanismi di controllo della postura strutturando programmi riabilitativi appropriati ed in linea con le teorie sul controllo motorio.
