Attività sportiva nel paziente portatore di ICD Nei soggetti portatori di defibrillatore impiantabile con funzione cardiaca normale o solo lievemente compromessa, nonché in quelli senza evidenza di cardiopatia strutturale, potrà essere concessa l idoneità sportiva solo per attività agonistiche ad impegno cardiovascolare minimo-moderato del gruppo B2 e non agonistiche del gruppo A (esclusi sindrome del QT lungo congenita, ARVD, TV catecolaminergica). Sports and depression/suicidal ideation (877 participants in the Nicotine Dependence in Teens study) 8% of young adults (mean age 20.4±0.7; 46% male) reported suicidal ideation in the past year Participation in sports teams in and (or) outside of school protected against suicidal ideation (OR 0.6; 95% CI 0.4 to 0.8; P = 0.002). Rischio di lesioni dovute alla momentanea perdita di controllo conseguente all aritmia o allo shock dell ICD apparentemente abbastanza basso Danneggiamento del dispositivo e/o degli elettrodi per trauma diretto o in conseguenza dei movimenti ripetuti delle braccia i dati generali riportano un incidenza di frattura dell elettrodo dello 0,4-4%. La pratica di un attività sportiva è probabile che possa significativamente incrementare tali percentuali
Conclusioni L attività sportiva è assolutamente controindicata nei pazienti affetti da una significativa cardiopatia strutturale Escludendo i pazienti in cui l aumento dell attività simpatica correlata allo sforzo fisico ha un effetto favorente sulle aritmie, può essere autorizzata un attività sportiva che non comporti uno impegno cardio-vascolare strenuo (evitare di superare la frequenza cardiaca target) Devono comunque essere evitati gli sport in cui può verificarsi un danneggiamento meccanico del sistema e quelli in cui una improvvisa perdita di coscienza può determinare un pericolo per il paziente o per chi gli è vicino (alpinismo, sci d acqua, immersioni etc.)
