2 SUMMARY Much is still unknown about the functions of sleep. The two process model of sleep regulation represents the most important theoretical basis for the study of sleep. Research conducted in recent years on sleep mechanisms has led to significant advances in the understanding of the relationships between sleep and physio and pathological conditions, with important diagnostic and therapeutic implications. The present thesis addresses basic questions of physiological sleep regulation, as well as the interaction between sleep and two pathologies, epilepsy and hepatic encephalopathy. Chapter 1 provides an introduction to the physiology of sleep, as well as the pathophysiology of nocturnal frontal lobe epilepsy and liver cirrhosis. In Chapter 2, the techniques applied for the analysis of the electroencephalographic (EEG) signals are summarized, with emphasis on period amplitude analysis, spectral decomposition and electrical source imaging. In the study presented in Chapter 3, power spectral analysis and period amplitude analysis were combined (derivation C3A2), providing a systematic and detailed investigation of the effects of sleep deprivation on slow oscillations in healthy subjects in the framework of the twoprocess model of sleep regulation. Slow oscillations were found to be characterized by increased frequency and amplitude under increased

3 sleep pressure. These changes suggest a faster alternation between upand down states at a cellular level. The study presented in Chapter 4 is an investigation on the topography of spectral EEG power and the intracranial distribution of electrical sources of slow oscillations during normal sleep and after sleep deprivation. The aim was to elucidate spatial characteristics of the homeostatic regulation of slow oscillations. Slow oscillations (0.5 1 Hz) were found not to be affected by sleep deprivation, whereas low delta activity ( Hz) was affected. A difference in the distribution of intracerebral sources between the two bands was found. Moreover, both bands displayed trait like behavior. In the study presented in Chapter 5, the electrical sources of delta activity (1 4 Hz) and sigma (12 16 Hz) during sleep were investigated in patients with drug resistant nocturnal frontal lobe epilepsy, in order to extract relevant clinical information on the lateralization of the epileptogenic zone, and at the same time to unveil the mechanisms regulating the mutual interaction between sleep and epilepsy. Promising results highlighted asymmetries in the source distribution of the two activities. This might be useful for clinicians in defining which hemisphere the epileptogenic focus is located in. In the last study (Chapter 6), we investigated the clinical, psychometric, wake /sleep EEG correlates of induced hyperammonaemia in healthy subjects and patients with cirrhosis. Increased ammonia concentrations were found to be associated with increased subjective sleepiness and a decreased ability to produce slow wave activity in patients, pointing to a ii

4 hyperammonaemia related impairment of the mechanisms regulating the alternation between different states of vigilance. Concluding remarks and a discussion in a broader context are provided in Chapter 7. In summary, the investigation of this thesis allowed to gain more insight into the homeostatic regulation of sleep slow oscillations in the healthy population, with particular attention to spatial aspects. The analysis of sleep during the ictal phase in patients with nocturnal frontal lobe epilepsy points to a potential of the analysis of sleep EEG to gain clinically relevant information on the characteristics of the disease. The investigation of sleep in a population of patients with cirrhosis highlighted the detrimental effect of blood ammonia levels in the ability to produce restorative sleep. iii

5 SOMMARIO La biologia del sonno rappresenta un aspetto della vita degli organismi del quale non si conoscono con certezza le funzioni. Il modello dei due processi di regolazione del sonno rappresenta la più importante base teorica per la comprensione dei meccanismi di regolazione dell alternanza sonno veglia. Negli ultimi anni, importanti scoperte hanno reso possibile la comprensione dei meccanismi che governano l interazione tra il sonno e diverse patologie, con implicazioni rilevanti in termini diagnostici e terapeutici. Questo lavoro di tesi ha come obiettivo lo studio della fisiologia del sonno, così come dell interazione di questo con l epilessia e la cirrosi epatica. Il capitolo 1 introduce le nozioni di base sulla fisiologia del sonno, e la patofisiologia dell epilessia notturna del lobo frontale e della cirrosi epatica. Nel capitolo 2 sono presentate le principali tecniche usate nell analisi del segnale elettroencefalografico (EEG), con maggiore enfasi sulle metodologie utilizzate nel lavoro di tesi. Il capitolo 3 è uno studio degli effetti della deprivazione di sonno sulle onde lente elettroencefalografiche che caratterizzano il sonno non REM di soggetti sani ( non rapid eye movement sleep ). L analisi spettrale è stata

6 combinata con un analisi periodo ampiezza delle onde. A causa dell aumentata pressione del sonno, le oscillazioni lente sono caratterizzate da frequenze ed ampiezze più elevate. Tale aumento suggerisce un alternanza più rapida tra depolarizzazioni ed iperpolarizzazioni a livello delle cellule corticali durante il sonno non REM. Lo studio presentato nel capitolo 4 è un espansione dell analisi precedente alla topografia ed allo studio delle sorgenti elettriche delle onde lente del sonno non REM, in condizioni basali e dopo deprivazione di sonno. Lo scopo dello studio è di evidenziare le caratteristiche spaziali della regolazione omeostatica delle oscillazioni lente. Le onde con frequenza compresa nell intervallo Hz non subiscono cambiamenti con l aumento della pressione del sonno, mentre quelle con frequenza nell intervallo Hz aumentano in determinate aree. Le due bande presentano differenze nella distribuzione tridimensionale delle sorgenti elettriche. Esse inoltre presentano caratteristiche distintive per ogni singolo individuo, come le impronte digitali. Il capitolo 5 è un analisi delle caratteristiche in frequenza del sonno in soggetti con epilessia notturna del lobo frontale. Lo scopo dello studio è di ottenere informazioni spaziali con rilevanza clinica in pazienti con farmacoresistenza, nei quali si pianifica un intervento chirurgico per la rimozione dell area epilettogena. I risultati dell analisi evidenziano un asimmetria dell attività elettrica durante il sonno non REM nelle bande di frequenza sigma (12 16 Hz) e delta (1 4 Hz), con una possibile applicazione diagnostica. vi

7 L ultimo studio (capitolo 6) è un analisi dei correlati clinici, psicometrici ed elettroencefalografici dell iperammoniemia indotta in soggetti sani ed in pazienti con cirrosi epatica. L aumento della concentrazione di ammonio e associato ad un aumento della sonnolenza soggettiva, sia nei pazienti che nei soggetti sani; inoltre nei pazienti si evidenzia una ridotta capacità di generare onde delta (1 4 Hz). Questa osservazione suggerisce un effetto nocivo dell ammonio sui meccanismi di regolazione dell alternanza sonno veglia. Infine nel capitolo 7 viene presentata una discussione generale dell intero lavoro. In conclusione, il lavoro di tesi ha consentito di acquisire maggiori conoscenze sulla regolazione omeostatica del sonno nei soggetti sani. Lo studio nei soggetti con epilessia notturna del lobo frontale ha mostrato una possibile applicazione dell analisi delle caratteristiche del sonno nella valutazione prechirurgica di tali pazienti. Lo studio nei pazienti con cirrosi epatica ha messo in luce uno dei meccanismi responsabili della scarsa qualità del sonno in questa categoria di pazienti. vii
