We will answer any questions about the form when you apply for the visa in person. 3) Passport or Travel Document with a validity that exceeds the intended stay of the applicant by at least three months. The passport must have a blank page to affix the visa; 4) Photocopy of Passport or Travel Document (main page and signature page only) 5) (For Non-US Citizens): Alien Resident Card or valid visa for residence and employment or study in the United States, with relevant documentation permitting reentry into the United States. (If sending application by mail, include ONLY THE NOTARIZED PHOTOCOPY of Alien Resident Card) 6) (For Non-US Citizens): Photocopy of Alien Resident Card or valid visa 7) Driver License or State I.D. as proof of residence in our jurisdiction (If sending application by mail, include ONLY THE NOTARIZED PHOTOCOPY of Driver License) 8) Photocopy of Driver License or State I.D. 9) Verification of employment: This should be a recent statement on company letterhead from employer stating position held, length of employment, recent salary and duration of vacation. **Job offer letters or pay-stubs cannot be accepted.** a. If you are self-employed or the owner of a company you must bring the following documents as verification of employment: 1) Copies of your business license (if applicable) 2) Original Business income tax returns 3) Photocopies of income tax returns 10) Proof of medical insurance showing minimum coverage of at least $50,000.00 covering the entire period of stay in Italy. US health insurance policies are accepted only with statements from the health insurance company confirming that the company will pay Italian/European providers. In lieu of this applicants may show proof of purchase of medical travel insurance for the whole travel period. 11) Current bank statement with applicant s full name, or notarized letter from the bank attesting to the applicant s financial status, or traveler checks. If presenting travelers checks please be sure to include the originals AND a photocopy. If using electronic statements please make sure that they include the name of the account holder. a. If you are supported by a spouse or relative please enclose his/her employment and financial information: employment letter, bank statement, and a notarized statement with which the person assumes financial responsibility for your stay in the Schengen area. 1
12) Proof of lodgings. This should include: a. Proof of confirmed hotel reservations for the entire duration of the applicant s stay in all Schengen countries visited; 1) If staying with a family or friend, the applicant MUST include the completed host invitation form (see attached form entitled Lettera di Invito). 1. If the host is not an Italian citizen, please include a copy of his/her permesso di soggiorno (residence permit in Italy). 2. If the host is an Italian citizen, please include a copy of the host s Italian passport, including the page showing the signature of the passport holder. 13) Proof of Flight reservation for roundtrip flights from the US to Europe (and ultimately to Italy) and back including internal entry and exit points of travel i.e. EURAIL tickets, car rental, internal flights (original plus photocopy) and a statement detailing your complete itinerary. The name on the reservations must reflect the name on the passport. The applicant may be required to show the actual ticket when picking up the visa. a. IF A MINOR IS TRAVELING TO ITALY AS A TOURIST he/she must have the consent of both parents. His/her birth certificate must be presented and his/her application must be signed by both parents. If the minor is not traveling with both parents, the parent who is not traveling must sign a separate, notarized, authorization form. PLEASE NOTE: THE PARENTS MUST BE THE SAME AS THE ONES SHOWN ON THE BIRTH CERTIFICATES. IF THEY ARE NOT, THE PARENT SHOWN ON THE BIRTH CERTIFICATE MUST GIVE HIS/HER CONSENT. 2
