Abstract Volume 118(1) July 2000 pp 61-65 Maximum Insufflation Capacity* Objective: To investigate Kang, Seong-Woong the effect of MD, deep PhD; lung Bach, insufflations John R. MD, on maximum FCCP insufflation capacities (MICs) and peak cough flows (PCFs) for patients with neuromuscular disease. [Clinica l Investigations: Pulmonary Function] Method: Forty-three patients with neuromuscular disease were trained in stacking delivered volumes of air to deep lung insufflation and were prescribed a program of air stacking once their vital capacities (VCs) were noted to be < 2,000 ml. VC, MIC, and unassisted and assisted PCF were monitored. The initial data were compared with the highest MICs subsequently achieved. For those patients whose MICs only decreased, we compared the initial data with the most recent data. Results: The MICs increased from (mean ± SD) 1,402 ± 530 ml to 1,711 ± 599 ml (p < 0.001) for 30 patients and only decreased for 13 patients. Patients for whom the MICs increased also had a significant increase in assisted PCF from 3.7 ± 1.4 to 4.3 ± 1.6 L/s (p < 0.05) despite having somewhat decreasing VCs and unassisted PCFs. Conclusion: With training, the capacity to stack air to deep insufflations can improve despite progressive neuromuscular disease. This can result in increased cough effectiveness. INSUFFLAZIONE 1. Si richiede al paziente di effettuare una massima inspirazione. 2. Apnea tele inspiratoria 3. Insufflazione con pallone Ambu (boccaglio o maschera) fino al massimo volume tollerato 4. Possibile ripetere l insufflazione più volte qualora la prima non sia sufficiente a raggiungere la Massima Capacità di Insufflazione (Air stacking)
Insufflazione con ventilatore Modalità volumetrica non invasiva: non è necessario variare l impostazione del ventilatore. Il paziente impara a non espirare per 2 o più respiri consecutivi fino ad arrivare alla MIC. Modalità pressometrica sia invasiva che non invasiva: è indispensabile variare il settaggio della pressione di inspirazione (pressione di picco almeno 40 cm H2O) e il tempo inspiratorio (almeno 3 ). Modalità pressione controllata.
RESPIRO GLOSSO-FARINGEO (GPB) E una tecnica non invasiva per assistere la ventilazione alveolare in pazienti con debolezza dei muscoli respiratori (Insuff. Resp. Restrittiva) Richiede l integrità della muscolatura glossofaringea e laringea (NO paralisi glottica). E una forma di respirazione a pressione positiva intermittente.
Valutazione efficacia della insufflazione Vital Capacity VC = massima quantità di aria che il paziente è in grado di espirare con la muscolatura disponibile Maximum Insufflation Capacity MIC = VC + aria insufflata che il paziente è in grado di trattenere a glottide chiusa prima di espirare. Lung Insufflation Capacity LIC = Massima quantità di aria che è possibile immagazzinare nei polmoni (unica insufflazione e misurazione del volume senza necessità di trattenere il respiro.
