Uomo, 50 anni Peso 78 kg altezza 1,75 m ANAMNESI: Non familiarità per TEV TVP femoro-poplitea dx 3 anni fa dopo aver fatto la maratona di Roma; in tale occasione fu trattato con Warfarin per 6 mesi con ottimo TTR (70%)
CUS arti inferiori di 3 anni fa
Uomo, 50 anni Peso 78 kg altezza 1,75 m ANAMNESI: Non familiarità per TEV TVP femoro-poplitea dx 3 anni fa dopo aver fatto la maratona di Roma; in tale occasione fu trattato con Warfarin per 6 mesi con ottimo TTR (70%) Circa 6 mesi fa recidiva di TVP poplitea sx successiva a lieve distorsione della caviglia dopo partita di pallacanestro
Al controllo del 6 mese la CUS dimostrava completa ricanalizzazione del vaso e il d-dimero era risultato negativo ai primi 3 controlli successivi Òòkk
Uomo, 50 anni Peso 78 kg altezza 1,75 m ANAMNESI: Non familiarità per TEV TVP femoro-poplitea dx 3 anni fa dopo aver fatto la maratona di Roma; in tale occasione fu trattato con Warfarin per 6 mesi con ottimo TTR (70%) Circa 6 mesi fa recidiva di TVP poplitea sx successiva a lieve distorsione della caviglia dopo partita di pallacanestro Giunge nel nostro ambulatorio per controllo dopo trattamento con Dabigatran 150 mg 1 c x 2 per 6 mesi Viene eseguita la CUS che dimostra completa ricanalizzazione del vaso e il d-dimero risulta negativo Il paziente ci comunica che vorrebbe sospendere il farmaco.
A questo punto del trattamento è corretto fare: a) Screening per trombofilia? b) Screening per cancro? c) Entrambi?
Thrombophilia screening continues to have little relevance in clinical decision making for anticoagulation. Prolonging anticoagulation beyond 6 months in an at-risk population decreased the risk of VTE recurrence regardless of their thrombophilia status.
