Methods Between 990-202, 268 patients underwent the Ross or Ross/Konno operation in 0 Italian pediatric cardiac units. Median age at operation was 0.2 years (2 days to 8 years). Indication was aortic stenosis with or without subaortic stenosis (n=00), aortic regurgitation (n=00), combined disease (n=68). Ninety-five (35%) children had history of prior cardiac surgery. Ross root replacement was performed in 79 patiens, inclusion cylinder in 3, sub-coronary grafting in 59 and Ross/Konno procedure in 58 patients. Descriptive and Kaplan-Meier analysis defined outcome, while Cox regression identified risk factors. Results There were 0 (3.7%) early and 2 late deaths, during a median follow-up of 7.2 years ( month-22 years). Survival was 93,2±,6%, 9,4±,9% and 82,2±8,8%, at 5, 0, and 5 years respectively. Freedom from any left heart reoperation was 96,5±,2%, 89,5±2,4% and 48,3±4,0%, at 5, 0, and 5 years respectively. Overall 28 right-heart sconduit were replaced (freedom from redo 6±3,6% at 5 yrs). Age lower than 4 months (p=0,000) and subcoronary implant technique (p=0,04) were identified as risk factors for death at multivariable analysis. The majority (76%) of late survivors were in NYHA class I with a regular sport activity. The most of patients weren t assuming anticoagulation therapy and about 2,6% of women had pregnancy. Conclusions Ross operation is associated with good survival and satisfactory autograft durability in children and adolescents. When performed in neonates and young infants (< 4 months), the Ross or Ross-Konno carries incremental hospital risk and should be delayed whenever feasible.
C39 CONNESSIONE CAVOPOLMONARE TOTALE ASSISTITA MECCANICAMENTE CON UNA POMPA ASSIALE: STUDIO COMPUTAZIONALE ED ESPERIMENTO IN VIVO F. Gandolfo, G. Brancaccio, S. Filippelli, E. Iannace 2, D. Amodeo Ospedale Pediatrico Bambino Gesù - IRCCS, Roma, 2 Ospedale Pediatrico Bambino Gesù - IRCCS, Taormina, 3 Università Cattolica Sacro Cuore - Policlinico Gemelli, Roma, ITALY Scopi della ricerca Oltre il 40% dei pazienti con fisiologia univentricolare e connessione cavo-polmonare totale (CCPT) vanno incontro ad un fallimento tardivo del sistema di Fontan. Il fallimento puo ascriversi o ad una insufficienza del ventricolo sistemico o ad un fallimento della circolazione di Fontan. Si rende quindi necessario il bisogno di nuovi presidi terapeutici considerando anche la limitazione di disponibilità di donatori per trapianto cardiaco nella popolazione pediatrica. Abbiamo quindi testato, prima con modelli di fluidodinamica computazionale (FDC) e poi su modello ovino, un prototipo di pompa assiale CHILD JARVIK 2000 al fine di sostenere la circolazione polmonare. Metodi impiegati La prima fase ha visto la creazione di un modello in vitro in grado di riprodurre l anatomia della CCPT. La pompa assiale veniva poi inserita per simulare le diverse modifiche nei flussi studiate con la FDC e quindi indagare la variazione di comportamento del sistema al variare della velocità rotazionale del device, nonché testare il miglior punto di inserimento della pompa nel circuito. Ciò ha costituito il background ideale per passare alla seconda fase dello studio cioè la creazione di CCTP nella pecora e poi l inserimento della pompa per analizzarne il comportamento in vivo. Sono state studiate 8 pecore ( 42-45 Kg). Dopo 2 esperimenti pilota, 6 pecore sono state sottoposte per tre ore a CCTP prima senza assistenza ( gruppo I) e poi le stesse pecore sono state assistite con supporto del device child Jarvik (gruppo II), che è capace di sviluppare flussi fra e 3 L/min in un range fra i 5000 e 9000 rpm. Nel caso di CCTP assistita con device un tubo di Goretex di 6 mm è stato interposto fra le due vene cave e connesso a T alla arteria polmonare mediante un graft composito che include in sequenza un a tubo di Goretex di 6 mm, la pompa assiale (posizionata il più distalmente possibile) e un piccolo segmento da 2 mm di Goretex. (fig ) Entrambi i gruppi sono stati svezzati dalla circolazione extracorporea. Nel gruppo assistito il device è stato regolato in maniera da mantenere la gittata cardiaca basale. Pressione, gittata cardiaca, resistenza vascolare sistemica e polmonare, i livelli di lattati e dell emogas analisi sono stati registrati per tre ore. Risultati e Conclusioni Nel gruppo non supportato (gruppo I) è avvenuto un progressivo e repentino deterioramento della funzione circolatoria con aumento dei lattati. Nel gruppo supportato (gruppo II) si è avuto invece un indice cardiaco stabile (2.85 ± 0.60 l/min/m2), pressione venosa sistemica di 0 ± mmhg e una pressione media in arteria polmonare di 2 ± mmhg con 2 mmhg di gradiente trans-device rilevato ad un indice cardiaco di 2.2 l/min. Le resistenze sistemiche e polmonari, i valori all emogas e i livelli di lattati sono rimasti stabili al valore basale. Questi dati hanno dimostrato come questa nuova pompa assiale garantisca un assetto emodinamico stabile nella CCPT. Questo modello animale sperimentale ha valutato la efficienza di questa nuova pompa pediatrica al supporto della circolazione di Fontan. <FILE IMAGE='664_203062525229.jpg'>
C40 PEDIATRIC CORONARY ARTERY REVASCULARIZATION: A EUROPEAN MULTICENTER STUDY V. Vida, G. Torregrossa, A. Guariento, M. De Franceschi, M. Padalino, G. Stellin Pediatric and Congenital Cardiac Surgery Unit, Dept. of Cardiac, Thoracic and Vascular Surgery, Padova, 2 Statistic Consultant, Dept. of Cardiac, Thoracic and Vascular Surgery, Padova, ITALY On the behalf of the European congenital heart surgeons association (ECHSA) Objective: We sought to evaluate the hospital and mid-term results of different surgical revascularization techniques in pediatric patients within the European Congenital Heart Surgeons Association (ECHSA). Methods: From 973 to 20, 80 patients from 3 ECHSA centres underwent 65 pediatric coronary artery bypass grafting (PCABG) and 27 other coronary artery procedures (OCAP)(2 patients combined PCABG and OCAP). Excluded were patients with Kawasaki disease. Median age at the time of coronary procedure was 2.3 years (range 2 days 6.9 years); 33 patients (4.2%) were younger then 2 months. An emergency procedure was necessary in 34 patients (42.5%). Results: Twelve patients (5%) died in hospital; age at surgery (p=0.02) and the need for an emergent procedure (p=0.0004) were related to hospital mortality. Median follow-up time was 7.6 years (range 0.9 23 years). There were 3 late cardiac deaths, all after a median time of 4 years (range 9 months-8.8 years) from PCABG. Fourteen patients (20.5%) presented with symptoms, including congestive heart failure (n=0) and angina (n=4) that were significantly associated to a low ejection fraction (p<0.00) and the presence of moderate/severe mitral valve regurgitation (p=0.0003). Six patients underwent a re-intervention for impaired myocardial perfusion; all of them had a stenotic/atretic PCABG (p=0.00) and the majority were symptomatic (5/6 pts, 83.3%, p=0.00). Conclusions: PCABG and OCAP are suitable surgical options in pediatric patients with impaired myocardial perfusion which increases operative and mid-term survival. Such population of patients needs to be followed for life to prevent/treat any possible cause of further myocardial ischemia.
C4 3 YEARS EXPERIENCE WITH ARTERIAL SWITCH OPERATION IN TRANSPOSITION OF GREAT ARTERIES: CONFIRMATIONS AND NEW PROSPECTIVES L. Arcieri, M. Melo, V. Pak, M. Cantinotti 2, M. Bernabei, R. Terapia Intensiva Pediatrica, FTGM, Massa, ITALY Background Arterial switch operation (ASO) is the surgical approach of choice for d-transposition of the great arteries (d-tga). Despite improvement in early and long term results, the incidence of adverse events is still considerable. In order to evaluate the operative results and midterm outcome in this population of patients we reviewed our experience with ASO for d-tga, with focus on the management and changes in the myocardial protection and neoaortic valve function. Patients and Results From january 2000 to january 203, 40 arterial switch operation were performed at our institution. Male were 98 (70%)and 42 were female(30%). Median age was 2,2 days (min 3 max 90) and mean weight was 3,43 kg(min,2 max 5,). VSD was associated in 52(37%)patients. Coronary anatomy was Yacoub type A in 97(70%) patients. Preoperative palliative procedure were Rashkind ballon septostomy in 84(60%)and pulmonary artery banding for left ventricular retraining in 5(0,5%). All cases were performed with extracorporeal circulation. Cardiopulmonary bypass time was 207,46 min and mean aortic cross clamp time was 25,94 min. Sternum was left opened in 2(80%) patients. Intubation time was 50 hours( min 25 max 384)and lenght of intensive care stay was 3 days (min. 3 Max 65). Overall hospital mortality was 5,7%. Since 2003(09 pts) hospital mortality has been 2,7%. Late mortality was 0,7%. Coronary complications requiring reoperation on the coronary arteries were 4(2,9%). Operation on the neoaortic or neopulmonary root were done in 2 (,4%) patients at a mean age of 4 months.follow up was complete for 00% of patients. Mean follow up time was 52 months(min max 54). Neoaortic regurgitation was mild in 50(35%)patients and moderate in 5 (3,5%)patients. More than 00 patients have undergone coronarography with a pattern of normality > 95%. Conclusions: ASO is the procedure of choice for TGA with good early and late outcome in terms of overall survival and freedom of reoperation. Myocardial protection and long term function of neoaortic valve remain the dominant issue in these setting. Development of a normal coronary three seems to be the rule. Reoperation are rare and quality of life is optimal.
C42 EVOLVING STRATEGIES FOR PRESERVING THE PULMONARY VALVE DURING EARLY REPAIR OF TETRALOGY OF FALLOT: MID-TERM RESULTS A. Guariento, V. Vida, B. Castaldi 2, M. Sambugaro 3, M.A. Padalino, F. The PV preservation offers an additional value to our standard early trans-atrial/trans-pulmonary repair by preserving both PV and RV function in the mid-term. Methods: Since January 2007, 69 patients who underwent early trans-atrial repair were enrolled. We divided patients into 2 groups: PV balloon dilation with preservation of PV annulus integrity (Group-) and PV reconstruction after annular incision (Group-2). Results: 34 patients underwent a successful PV annular preservation (49%). Median age at surgery was 3 days (range 36-52 days)(group- vs Group-2, p=ns). Median preoperative PV Z-score was -3.5 (range -0.95 to -5.62) (Group- vs Group-2, p=0.03). Median ICU and hospital stay were respectively 3 and 0 days (Group- vs Group-2, p=ns). Median follow-up time was 580 days (range from 89 to 940 days)(group- vs Groups-2, p=0.08). Two patients were re-operated for residual RVOT obstruction ( in Group- and in Group-2). The remaining patients are alive and well. Median peak systolic RVOT gradient is 25 mm Hg (range 8-60 mm Hg)(Group- vs Group-2, p=ns). The degree of PV regurgitation was none/mild in 24 patients (80%) and moderate in 6 (20%) in Group-; was none/mild in 8 patients (25%), moderate in (34.4%) and severe in 3 (28.6%) in Group-2 (p<0.000). Median right ventricular fraction area change was 55% (range 42-70%) in Group- and 50% (range 40-63%) in Group-2 (p=0.003). Conclusions: The integrity and function of the PV can be preserved in selected patients during early repair of TOF by concomitant balloon dilation. We believe that this method can contribute to avoid long standing RV dysfunction, and possibly future interventions for treating PV regurgitation.
C43 IMPATTO DELLA CHIRURGIA MITRALICA ASSOCIATA AD INTERVENTO DI ROSS-KONNO SULL OUTCOME A BREVE E MEDIO TERMINE F. Gandolfo, G. Brancaccio, S.B. Albanese, A. Amodeo, E. Cetrano, D. Di Carlo, G. Carotti Ospedale Pediatrico Bambino Gesù - IRCCS, Roma, ITALY Scopi della ricerca Nei casi più severi di ostruzione all efflusso ventricolare sinistro in età pediatrica, come nella Sindrome di Shone, oltre alla operazione di Ross-Konno (ORK) è necessario agire chirurgicamente e contemporaneamente anche sulla valvola mitrale. Ciò pone tre valvole cardiache potenzialmente a rischio di danneggiamento con il conseguente dubbio, in tali casi estremi, se sia meglio perseguire la strada della palliazione a singolo ventricolo. In letteratura in effetti è riportata una più alta mortalità e maggiore frequenza di reinterventi quando è associata alla ORK anche chirurgia mitralica. Scopo dello studio è pertanto quello di valutare l impatto sui risultati a breve e medio termine della procedura chirurgica sulla valvola mitrale (CVM) combinata alla ORK per il trattamento di cardiopatie congenite complesse con ostruzione del tratto di efflusso ventricolare sinistro. (OTEVS). Metodi impiegati Da Novembre 993 a Dicembre 202, 3 pazienti con OTEVS complessa sono stati sottoposti a ORK nella nostra istituzione. In 3 casi (42%) è stato necessaria la CVM: per 0 di essi la stenosi mitralica era dovuta ad ipomobilità del lembo posteriore della mitrale (LPM) e muscolarizzazione delle strutture sottovalvolari. Le tecniche di CVM sono state: resezione di tessuto in eccesso ( 5 casi, 36%), commissurotomia ( 4 casi, 28%) ed aumento del LPM con patch ( 3 casi, 22%) e altra tecnica (2 casi, 4%). I pazienti sono stati poi seguiti in follow up per un intervallo medio di 6,03 anni ( 4 giorni 2.04 anni). Risultati e Conclusioni Nel gruppo ORK + CVM (età mediana di 9,4 mesi; peso mediano di 9,4 Kg) ci sono state 2 morti perioperatorie (5%), mentre 4 decessi (22%, 2 perioperatori e 2 ad un mese dalla chirurgia) sono occorsi nel gruppo ORK (età mediana 23,6 mesi; peso mediano di,5 Kg) (p=ns). La libertà a sei anni da reintervento per sostituzione del condotto è stata del 78% nel gruppo ORK e 85% in quello ORK + CVM (p=ns). In esso, inoltre, la libertà a sei anni per nuova procedura sulla valvola mitrale è stata dell 84%. Nessun paziente ha avuto ricorrenza di OTEVS, né insufficienza dell autograft. In conclusione, quindi, nella nostra casistica di singolo centro, la ORK per l OTEVS complessa con associata chirurgia mitralica in tempo unico non rappresenta un fattore di rischio aggiuntivo in termini di mortalità o necessità di reintervento per ogni causa, a breve e medio termine. Pertanto la ORK con associata CVM rappresenta una valida opzione terapeutica che può essere perseguita in questo delicato subset di pazienti.
