ISS - SNLG Per i pazienti epatopatici cronici che possono sviluppare epatocarcinoma è raccomandato l impiego della US di base. In caso di esito positivo o dubbio o di paziente ad elevato sospetto di malattia, l esame deve essere seguito da TC o RM con mezzo di contrasto epatospecifico ( Chen TH et al.,int J Cancer, 2002) ( Trevisani F et al., Am J Gastr, 2004 )

ISS - SNLG Per la caratterizzazione di lesioni focali del fegato e per la conferma diagnostica, cioè di secondo livello, di lesioni focali del fegato altrimenti diagnosticate è raccomandato l uso della CEUS Per i casi in cui la CEUS non dia risultati adeguati e definitivi è raccomandato l uso della RM con mdc epatospecifico. Per i pazienti che si trovano in particolari situazioni per le quali non è possibile utilizzare né la CEUS né la RM, è raccomandato l uso di TC spirale (Wang JH et al., Liver Int, 2006) (Farraher SW et al.,j Magn Reson Imaging, 2006)
ISS - SNLG Anche se le prove attualmente disponibili non consentono di formulare reali raccomandazioni in merito alla definizione del ruolo delle diverse tecniche di imaging US, CEUS,TC,RM e PET nella stadiazione locoregionale delle neoplasie epatiche primitive, è tuttavia opinione degli esperti che sia possibile consigliare l uso della TC e della RM nella stadiazione locoregionale, limitando l uso delle tecniche invasive ( IOUS e CE-IOUS) solo come supporto ad un eventuale intervento.

CEUS non contemplata nella AASLD practice guideline ( Hepatology, July 2010)..CEUS is not available in the USA..in 1-2 cm. nodules the typical vascular pattern with the imaging techniques ( CEUS, CT, MRI) has absolute specificity for HCC ( Sangiovanni A, Gut, 2010 )!!..false positive for HCC have been described for CEUS in patients with proven ICC!! ) ( Vilana R, Hepatology, 2010 ) Reclutamento 2003-2009 : omogeneità di esecuzione di MR?!? A hypovascular lesion < 2 cm with a vaguely nodular lesion cannot be accurately diagnosed on imaging Such lesions should undergo guided core biopsy tumor cell invasion into the intratumoral portal tracts is the most important finding in differentiating early HCC from HGDN
EASL/EORTC 2012 ( J Hepatol 2012 )..a more cautions application of this rule is reccomended in suboptimal settings, where the technology at disposal or the local skills are not at the high-end level. In this circumstances we reccomend to use two coincidental techniques...the role of contrast-enhanced ultrasound is controversial.

Raccomandazioni AISF 2012 Crescenti evidenze indicano che la RM multifasica con mdc epatospecifico è superiore a CEUS e TC multifasica in termini di sensibilità ed accuratezza diagnostica per il piccolo HCC, soprattutto se ipovascolare ( Ahn SS et al., Radiology 2010) ( Golfieri R et al., Eur Radiol, 2011)
Conclusioni (?!?) Lo screening e la sorveglianza si fanno con l ecografia ( e αfp?) Nodulo in cirrosi (o HBV)< 1 cm. 3 mesi Nodulo in cirrosi (o HBV)> 1 cm. CEUS, TC multifasica, RM con mdc epatospecifico (!)
