Odontoiatria Anno XXXIII - N. 1 pp. 11-15, Marzo 2014 Valutazioni cliniche di un nuovo dispositivo interdentale nel mantenimento dei manufatti implantoprotesici Clinical evaluation of a new interdental flossing device for maintaining prosthetic implants Introduzione Uno degli obiettivi principali dell odontoiatria è da sempre il ripristino degli elementi dentari persi a causa di processi cariosi, malattia parodontale, traumi o assenti per agenesie. Da questa esigenza è scaturito un crescente interesse per gli impianti dentali che oggi hanno risultati riabilitativi sovrapponibili a quelli su denti naturali in termini di sopravvivenza, successo ed accettazione da parte del paziente a lungo termine. I tessuti molli perimplantari presentano una conformazione anatomica differente rispetto ai tessuti intorno al dente. Sul dente l attacco connettivale è garantito dalle fibre collagene che si inseriscono direttamente sul cemento radicolare, mentre il connettivo perimplantare non si attacca direttamente al titanio. Non è pertanto presente la resistenza incontrata dal tessuto connettivo che crea una barriera meccanica, ma è la mucosa perimplantare a creare una barriera contro l accesso di batteri. Inoltre il tessuto perimplantare presenta una notevole diminuzione della vascolarizzazione, se confrontato al tessuto parodontale, rendendolo più labile nei confronti del biofilm batterico ed alla sua induzione all infiammazione non riuscendo ad arginare i mediatori di quest ultima soprattutto nelle importantissime fasi di riparazione tissutale. È quindi di fondamentale importanza preservare i tessuti perimplantari dalla colonizzazione batterica soprattutto di alcuni ceppi quali Porphyromonas gingivalis, Actinobacillus Actinomycetemcomitans e Prevotella Intermedia, pur ricordando che anche la genetica gioca un ruolo importante nello sviluppo di mucositi e perimplantiti. Introduction One of the main goals of dentistry has always been the restoration of teeth lost as a result of dental caries, periodontal disease, trauma, or absence due to tooth agenesis. This need has brought about a growing interest in dental implants which today show clinical results comparable to those of natural teeth in terms of survival rate, success rate and acceptance by the patient in the long term. Peri-implant soft tissues are anatomically different from the tissues around the tooth. The connective tissue on the tooth guarantees its attachment through the collagen fibres which are inserted directly in the root cement, whereas the peri-implant connective tissue does not adhere directly to titanium. Therefore the resistance encountered by the connective tissue which would normally create a mechanical barrier is not present, and the peri-implant mucous membrane has to create a barrier to prevent bacterial ingress. In addition, the peri-implant tissue presents a significant decrease of blood flow when compared to the periodontal tissue, making it more susceptible to bacterial biofilm attack and consequent inflammation and unable to resist it especially in the extremely important phases of wound healing. It is therefore of paramount importance to protect peri-implant tissues from bacterial colonization, especially from such strains as Porphyromonas gingivalis, Actinobacillus Actinomycetemcomitans and Prevotella Intermedia, while at the same time recognising that genetics also plays an important role in the development of peri-implant mucositis and peri-implantitis. Nella pratica clinica, nel contrastare queste patologie, la terapia antimicrobica sta assumendo sempre più importanza soprattutto nei riguardi della perimplantite: patologia per la quale ad oggi non esiste un definito protocollo terapeutico validato scientificamente od una metodica che presenti risultati più efficaci di altre. Il successo di una riabilitazione implantoprotesica è quindi determinato dal mantenimento nel tempo dell osteointegrazione. I fallimenti implantari sono rappresentati dal sovraccarico occlusale o da un origine infettiva, fenomeni che entrambi portano il riassorbimento dell osso perimplantare e la mobilizzazione della fixture. Risulta chiara l importanza delle manovre di igiene orale professionale e domiciliare nella rimozione della placca batterica dalle superfici implantari. Nonostante tutte le ricerche atte a rintracciare nuove tecnologie per il mantenimento professionale, la prevenzione primaria attraverso un mirato mantenimento domiciliare dell equilibrio batteriologico risulta il gold standard. Nei fallimenti ad eziologia infettiva, la diffusione dei batteri in profondità è rappresentato dalla comunicazione del cavo orale con il tessuto osseo profondo e dipende anche dall adesione della placca batterica alla superficie metallica implantare. La flora batterica riscontrata in casi di mancata osteointegrazione è sovrapponibile a quella che provoca la periodontite nell adulto. In pazienti che richiedono particolare attenzione all igiene orale potrebbe rappresentare una possibile alternativa al filo interdentale l utilizzo di uno strumento poco invasivo come l idropulsore per la rimozione di detriti alimentari interdentali. Inoltre questo strumento permette l irrigazione delle superfici orali con diversi agenti chimici. La buona riuscita della terapia implantare trova sussidio nella compliance del paziente che deve seguire in modo corretto le indicazioni date dall Igienista Dentale. Solo in questo modo il paziente potrà mantenere i suoi manufatti implantari in stato di buona salute. La ricerca si orienta verso strumenti che semplifichino le manovre di igiene orale domiciliare, capaci di essere confortevoli per il paziente affinchè l igiene degli spazi interdentali e la cura dell impianto non vengano trascurati. creasingly important in fighting these diseases, especially with regard to peri-implantitis: a condition for which there is not currently a defined scientifically validated treatment protocol or even a remedy with more effective results than any other. Clearly, oral hygiene is important - both professional and at-home care - in removing plaque from the surfaces of the implant. Despite extensive research has been done finding new technologies for professional maintenance, primary prevention by means of dedicated at-home hygiene to achieve bacteriological balance is up today the best method. In implants failures due by infection, serious bacterial spread is evident w and depends also on the adhesion of bacterial plaque to the metal surface of the implant. Bacterial flora found in cases where osseointegration has failed is comparable to that which causes periodontitis in adults. In patients where it is necessary to pay particular attention to oral hygiene, a possible alternative to flossing could be the use of a minimally invasive tool such as a dental water jet for the removal of food debris between the teeth. This tool also enables oral surfaces to be cleaned with different chemical agents. For implant therapy to be successful, the cooperation of the patient in following the istructions given by the Dental Hygienist is important. This is the only way for the patient to maintain their dental implants in a good state of health. Research is orientated towards tools that simplify at-home oral hygiene, making it comfortable for the patient so that interdental hygiene and care of implants are not neglected. Materiali e metodi L obiettivo dello studio è quello di valutare l effettivo valore di riduzione dell indice di placca batterica attraverso l utilizzo dell idropulsore Philips Sonicare (Philips, Amsterdam, The Netherlands) negli spazi interdentali dei manufatti implantari, poichè queste zone possono rappresentare nicchie ritentive per il biofilm batterico: i presidi finora usati come il filo interdentale e lo scovolino, spesso risultano non completamente efficaci. Materials and methods The aim of this investigation is to assess the actual value of the reduction in theplaque index through the use of the Philips Sonicare dental water jet (Philips, Amsterdam, The Netherlands ) in the interdental spaces between dental implants, since these areas are susceptible to the build-up of bacterial biofilm:interdental tools like as dental floss and toothpicks are often not sufficiently effective. Screening e selezione dei soggetti: Per effettuare questo studio sono stati selezionati un totale di 26 pazienti fra gli utenti che afferiscono all Istituto Stomatologico Toscano, Ospedale della Versilia, Lido di Camaiore (Lucca, Italia) per effettuare questo studio. Quindici (15) di sesso maschile, undici (11) di sesso femminile. Screening and selection of subjects: A total of 26 patients were chosen for this study from those attending the Dental Institute of Tuscany, Versilia Hospital, Lido di Camaiore (Lucca, Italy). Criteri di inclusione: - buono stato di salute generale, - assenza di patologie sistemiche, - presenza di un indice di placca superiore al 30%, - presenza di manufatti implantari, - assenza di un periodico controllo dall igienista dentale - scarse capacità manuali di rimozione della placca. Inclusion Criteria: - good general health, - absence of any systemic disorders, - plaque index above 30%, - presence of dental implants, - lack of regular maintenance visits done by a dental hygienist, - limited dexterity in plaque control with manual aids. Criteri di esclusione: - presenza di manufatti ortodontici, - uso di altri sussidi interdentali per il controllo della placca, - uso di medicinali che alterano lo stato di salute gengivale, - paziente non collaborativo, non disponibile a presenziare a periodici controlli Esclusion Criteria : - presence of orthodontic brackets - use of other interdental plaque control aids, - use of medications that affect the health status of gums, - patient not compliant, unavailable to attend regular checks Modalità studio: I pazienti sono stati suddivisi in maniera randomizzata in gruppo test, dove è stato utilizzato l per la rimozione della placca interdentale, e gruppo controllo. È stato richiesto il consenso a tutti i pazienti inclusi nello studio in oggetto. A tutti i pazienti coinvolti è stata effettuata una seduta di igiene orale professionale e sono state date le istruzioni per le procedure di igiene orale domiciliare con le seguenti modalità. Method: Patients has been divided randomly into a control group, and a test group where was used for the removal of interdental plaque. A consent has been asked to all patients included in the study To all patients involved in the study a professional oral hygiene procedure has been performed together with instructions for carrying out a correct at-home oral hygiene as follow: Gruppo Test: 1 spazzolare due volte al giorno per 2 minuti con il metodo di spazzolamento in uso dal paziente. 2 utilizzare l ponendolo in posizione verticale per favorire l effetto del getto in modo tale che il beccuccio vada ad appoggiarsi negli spazi interdentali dei denti naturali e dei manufatti implantoprotesici. Effettuare 3-4 spruzzi in ogni spazio. Test Group: 1. brush twice a day for 2 minutes using the brushing meth od already in use by the patient. 2. use the in an upright position. In this way the water jet tips will be placed in the interdental space of both natural teeth and dental implants. Spray 3-4 times into each space. Gruppo Controllo: 1 spazzolare due volte al giorno per 2 minuti con il metodo di spazzolamento in uso dal paziente. 2 nessun uso di altri sussidi interdentali per il controllo della placca La durata dello studio è stata di 30 giorni, gli indici vengono registrati. Ad ogni paziente sono stati rilevati, all inizio dello studio (T0) e dopo 30 giorni (T1) i seguenti indici parodontali: - l indice di placca batterica, utilizzando la tecnica (PCR) di O LEARY; Control group: 1. brush twice a day for 2 minutes using brushing method already in use by the patient. 2. no other interdental aids for plaque control were used. The study took place over 30 days, then results has been recorded by the dental hygienist. Risultati - Indice di placca: Il Gruppo Test ha mostrato un valore medio di PCR al T0 di 53,62% ed al T1 di 23,04% con una differenza del 30,58%. Il Gruppo Controllo ha mostrato un valore medio di PCR al T0 di 44,40% ed al T1 di 24,84% con una differenza del 19,56 (Tab. 1) - Sanguinamento al sondaggio: - Il Gruppo Test ha mostrato un valore medio di GBI al T0 di 31,79% ed al T1 di 12,94% con una differenza del 18,85%. - Il Gruppo Controllo ha mostrato un valore medio di GBI al T0 di 35,14% ed al T1 di 27,80% con una differenza del 7,34%.(Tab. 2) Results - Plaque Index: The Test Group showed an average PCR value of 53.62% at T0 and 23.04% at T1: with a difference of 30.58%. Conclusioni Lo sviluppo di protocolli di igiene orale non può prescindere da un accurata analisi della letteratura disponibile in linea con quanto dettato dall EvidenceBased Dentistry, per quando riguarda l uso di questo nuovo dispositivo uscito in commercio da pochi anni. L utilizzo di non è stato ancora ampiamente testato in un elevato numero di studi, le evidenze ad oggi disponibili sono pienamente concordi nell indicare come l irrigazione giornaliera con possa rappresentare un efficace supplemento allo spazzolamento quotidiano soprattutto per quanto riguarda il mantenimento domiciliare degli spazi interdentali e delle zone dei manufatti protesici difficili da detergere. Nel corso degli studi clinici, i pazienti che hanno utilizzato hanno mostrato una riduzione sia dei valori dell indice di placca che di sanguinamento. In particolare, l irrigazione con potrebbe rappresentare una possibile alternativa al filo interdentale nei pazienti con scarsa manualità e compliance. Conclusions The development of protocols for oral hygiene cannot discount a thorough analysis of the available literature in line with what is dictated by Evidence-Based Dentistry, insomuch as it relates to the use of this new device which has only been in commercial use for a few years. has not yet been extensively tested in long term trials, although to date, the evidence available shows that daily cleansing with can be an effective supplement to daily brushing, especially with regard to personal ne care of interdental spaces and parts of dental implants which are difficult to reach and to clean. In clinical trials already published, patients who used Air- Floss showed a reduction in both plaque and bleeding index values.
