APCs link innate and T-cell mediated immunity. Pathogens or necrotic/damaged tissues are detected by APCs, such as dendritic cells (DCs) and resident tissue macrophages, through pattern recognition receptors (PRRs) such as toll-like and scavenger receptors. Signal transduction by way of these receptors leads to DC maturation and activates a wide array of interconnected and synergistic host defense mechanisms. DC maturation involves upregulation of class I and II MHC molecules and costimulatory molecules, such as CD80 and CD86, and, therefore, enables the DCs to present antigen to T cells. DC maturation also induces the production of various cytokines that influence T-cell differentiation. PAMP, pathogen-associated molecular patterns; SRA, scavenger receptor-a.
The two-signal model of T-cell activation B) T cells that are stimulated through the TCR, after specifically binding to the antigenic peptide-mhc complex, and through CD28, after binding to B7 molecules (CD80, CD86) on APC, generate multiple biochemical signals that result in new gene transcription and augmented stability of mrnas that encode T-cell cytokines and cell surface molecules. T cells that receive two signals differentiate into helper or effector T cells. EXAMPLE: epithelial salivary cells express B7 in Sjögren s syndrome
Afferent costimulatory molecule pairs on T cells T-cell surface receptors of the immunoglobulin superfamily include CD28, ICOS, CTLA-4, and PD-1. CD28 and ICOS transduce positive costim. signals, while CTLA-4 and PD-1 negative signals to the T cell. CD28 is expressed constitutively, whereas ICOS, CTLA-4, and PD-1 are expressed after T-cell activation. CD28 and CTLA-4 share at least two ligands, CD80 (B7-1) and CD86 (B7-2). In the early phase of a T-cell mediated immune response, CD28 will bind preferentially to B7-2 and mediate positive costimulatory signals. Later in a T-cell mediated response, when CTLA-4 is expressed on the cell surface, CTLA-4 will compete with CD28 for binding B7-1 (expressed later) and B7-2, and T-cell activation will be modulated negatively. EXAMPLES: in RA: blocking co-stimulation: CD28 / T cells with CTLA4Ig NEJM 2004 in SLE: increased B7-2 on APCs Azathioprine: inhibits CD28-dependent T cell activation apoptosis
Efferent signals to targets of T cells by CD40 ligand-cd40 interactions. NB: negative regulators are also present on T cells: FasL, TNF, TRAIL, CD30L. (TNF/R family) Hyper IgM sn.for CD40Lmutations A) Interaction between CD40L on Th cells and CD40 on DCs results in expression of cytokines, including IL-12, and costimulatory molecules, including B7-1 and B7-2. B) Interaction between CD40L on Th cells and CD40 on B cells results in B-cell proliferation; increased expression of costimulatory molecules, including B7-1 and B7-2; and augmented capacity for antigen presentation; and immunoglobulin class switching to mature isotypes. C) Interaction between CD40L on Th cells and CD40 on monocytes results in expression of proinflammatory molecules, including TNF-α and nitric oxide. EXAMPLES IN SLE: increased CD40L, and role of anti-cd40l therapy RF: CD40-40L interaction is crucial for RF production by B cells
