SHOCK DAMAGE RESUSCITATION E' un insieme di provvedimenti rianimatori atti a mantenere il paziente vivo fino al successivo livello di cura Evitare triade letale (acidosi, ipotermia, emodiluizione con coagulopatia da consumo) Mantenere normo capnia ventilazione protettiva PaCO2 35-40 mmhg TV <6 ml/kg Mantenere PAS 80 mmhg Se trauma cranico PAM 80 mmhg Target Hb 7-9 gr/dl Hemostatic resuscitation o ECS protocol
PROTOCOLLO ECS ENTRO 30 MIN ENTRO 60 MIN Il paziente (età > 16 anni) presenta una emorragia significativa (o rischio di)? Prelievo per: gruppo sanguigno, emocromo, glucosio, K, Ca, Na, ALT, lipasi, creatinina, PT-INR, aptt, fibrinogeno, EGA arterioso con lattati + TEG basale (TEG 0) Richiesta per GRC urgente e plasma (da non scongelare) Assume antiaggreganti o anticoagulanti? Antagonizzare! Entro 3 ore dal trauma? Acido tranexamico: 1 g ev in 10 minuti + 1 g ev in 8 ore Presenta emorragia significativa + almeno 1 dei seguenti segni: pas < 100 mmhg be < - 6 lattati > 5 mmol/l hb < 9 g/dl (inr > 1,5) NO Se tash score > 15 e/o non risponde al test di riempimento SI! ATTIVARE PROTOCOLLO ECS-TRAUMA TEAM TRASFONDERE CON 4 UNITA 0 NEG + FIBRINOGENO 2 GR Completare valutazione calcolando tash score + test riempimento ORDINE DI DECONGELARE PLASMA IN ATTESA DEL PLASMA INFONDERE CRISTALLOIDI E/O COLLOIDI ALL ARRIVO DEL PLASMA SOMMINISTRARE PLASMA:GRC > 1:1,5 MANTENERE PIASTRINE > 100.000/MC Se tash score < 15 e/o risposta al test di riempimento TERAPIA IN FUNZIONE DEI DATI TEG 0 RIPETERE TEG OGNI 30-45 MINUTI

E. Vasoactive Medication Norepinephrine first-choice vasopressor Addition of vasopressin (up to 0.03 U/min) or epinephrine to norepinephrine with intent of raising MAP to target, or adding vasopressin (up to 0.03 U/min) to decrease norepinephrine dosage Dopamine alternative vasopressor only in highly selected patients (e.g.,low risk of tachyarrhythmias and absolute or relative bradycardia) No low-dose dopamine for renal protection Dobutamine used in persistent hypoperfusion despite adequate fluid loading and use of vasopressor agents (titration to end point perfusion, reduced or discontinued if worsening hypotension or arrhythmias)
