EPIDEMIOLOGIA Incidenza tra 3.5 e 7 per 100.000 abitanti Epidemiologia bimodale con massima incidenza tra i primi 2 anni e sopra i 75 anni Prognosi peggiore in bambini e immunodepresi Incremento con viaggi e migrazioni Several factors influence outcome, including the age of the patient, the level of consciousness at time of presentation and the duration of illness (Schmolck, Maritz, Kletzin, & Korinthenberg, 2005; Whitley & Gnann, 2002). The main influencing factor however, appears to be the aetiological agent involved. This is probably a reflection of not only the pathogenesis of the organism but also treatment available for specific pathogens and host factors that may come into play

Gradi di evidenza
EVIDENCE BASED MEDICINE Tipo di terapia Raccomandazione Livello di evidenza EMPIRICA EMPIRICA Acyclovir should be initiated in all patients with suspected encephalitis, pending results of diagnostic studies Other empirical antimicrobial agents should be initiated on the basis of specific epidemiologic or clinical factors, including appropriate therapy for presumed bacterial meningitis, if clinically indicated A III A III 10-15 mg/kg IV 4 volte al giorno per 2-3 settimane (circa 700-800 g per 4)
EVIDENCE BASED MEDICINE Tipo di terapia Raccomandazione Livello di evidenza Specifica HSV Herpes simplex virus: acyclovir is recommended A I Specifica HIV HIV: HAART is recommended A II Specifica non virale In patients with clinical clues suggestive of rickettsial or ehrlichial infection during the appropriate season, doxycycline should be added to empirical treatment regimens Specifica JCV JC virus: reversal of immunosuppression A III A III

TO BE CONTINUED
