Rivalutazione clinica dopo chemioterapia Modesta risposta in termini di riduzione volumetrica del tumore. Intervento chirurgico in presenza di fattori di rischio, ma con l intento di radicalità di asportazione. Cosenza, 25/11/ 2005
Intervento chirurgico (1) Massa solida ovalare ben capsulata, dimensioni cm 10x8x5, 4 minute formazioni nodulari accluse alla capsula. Localizzazione: paramediana, sovracavale. Versante superiore: adeso alla vena porta e alla via biliare extraepatica; Polo inferiore: aderente e sottostante al ginocchio inferiore del duodeno; Faccia inferiore: contigua alla fascia renale senza infiltrazione; Surrene indenne.
Intervento chirurgico
Intervento chirurgico (2) Asportazione radicale senza residui macroscopici. Valutazione estensione regionale del tumore mediante asportazione di linfonodi (leg. epato-duodenale, periportali, peritumorale, e perirenale) e strutture anatomiche contigue al tumore (connettivo peritumorale e peritoneo peritumorale).
Istologia Neuroblastoma a stroma povero differenziante a cellularità intermedia con basso indice mitotico mitotico/carioressico e con isolato focolaio di microcalcificazione. Indenni i margini di resezione chirurgica. Categoria prognostica INPC: favorevole. Linfonodi e strutture anatomiche biopsiate: esenti da neoplasia.
Decorso post-operatorio Regolare l immediato decorso, dimesso in settima giornata. Follow-up post-operatorio. Attualmente esente da malattia
