Giampiero Carosi Istituto di Malattie Infettive Università degli Studi di Brescia Organization models for the management of patients with Hepatitis B and C Secondo Workshop nazionale di economia e farmaci in epatologia WEF-E 2012 Roma, 2 febbraio 2012
Viremia 2. Resistenze 3. Fattori predittivi dell outcome della terapia 4. Safety of treatment 5. Therapeutic Drug Monitoring(TDM) 6. Aderenza alla terapia
1. Valutazione viremia per HBV Ideale: controllo off-drugs (20-30%) Realistico: controllo on drugs SHORT-TERM "CURATIVE TREATMENT Peg-IFN Follow-up (mo/yrs) HBV DNA < 2000 IU/mL HBV DNA < 2000 IU/mL ALT < UNL HBsAg Loss LONG-TERM "SUPPRESSIVE TREATMENT NUCs HBV DNA undetectable by PCR (<10-15 IU) HBsAg seroconv.
1. Valutazione viremia per HCV Telaprevir regimen in G1 HCV-infected treatment-naive patients Stop at week 24 if undetectable at week 4 and 12 (e-rvr) Telaprevir + PR Peg-IFN alfa + ribavirin Peg-IFN alfa + ribavirin if detectable at week 4 or 12* W 0 12 4 24 36 48 HCV RNA: If > 1000 IU/mL at Week 4 or 12: discontinue all drugs If detectable at Week 24 or 36: discontinue PR *In Phase III studies, a sensitive real-time PCR assay with a limit of quantification of 25 IU/mL and a limit of detection of 10-15 IU/mL was used to determine whether HCV RNA levels were undetectable. Detectable HCV RNA below the lower limit of assay quantification should not be used as a substitute for undetectable for making decisions on treatment duration, as this may lead to an insufficient duration of therapy and higher relapse rates.
1. Valutazione viremia per HCV Boceprevir regimen in G1 HCV-infected treatment-naive patients Stop at week 28 if undetectable at week 8 and 24 PR leadin BOC + PR If detectable at W 8 but undetectable at W 24: BOC + PR* PR* W 0 4* 8 12 24 28 36 48 HCV RNA: Assess for RGT criterion If 100 IU/mL discontinu all drugs If detectable discontinue all drugs * if HCV RNA decline < 1 log at week 4 (lead-in phase) continue therapy until 48 week
Using HBsAg to Predict Response to PegIFN alfa-2a in HBeAg-Negative Patients N = 102 Any HBsAg decline at Wk 12 No (n = 54) Yes (n = 48) HBV DNA decline at Wk 12 (c/ml) < 2 log (n = 20) 2 log (n = 34) < 2 log (n = 20) 2 log (n = 28) Proportion With SR* 0% 24% 25% 39% *SR = HBV DNA < 10,000 c/ml (~ 2000 IU/mL) and normal ALT at Wk 72. Results suggest one should consider pegifn discontinuation if no HBsAg decline and < 2 log HBV DNA decline at Wk 12 Rijckborst V et al, Am J Gastroenterol 2010, Hepatology 2010
Liver Biopsy in the era of non invasive methods Grading and staging necessary before starting treatment? + Possible use of non invasive methods for monitoring Castera L. et al, Gut 2010
Retrospective analysis of SPRINT-2 and RESPOND-2: randomized, phase III trials [1] IL28B genotype strongly predicted both pegifn/ribavirin lead-in (week 4 response) and Week 8 response (decision point for response-guided therapy) Response to 4-week pegifn/ribavirin lead-in stronger predictor of SVR regardless IL28B genotype [1] Poordad F, et al. 46th Annual Meeting of the European Association for the Study of the Liver; March 30 - April 3, 2011; Berlin, Germany. Abstract 12.
4. Safety Epatite B: Side-effects: Peg-INF >30% (very frequent) Anemia, Piastrinopenia, Rash 1-30% (frequent) Depressione Buster E. et al, Best Practice and Research Clinical Gastroenterol.
5. Therapeutic Drug Monitoring (TDM) Epatite B: TDM non necessario: non drug-drug interactions (DDIs) Epatite C: TDM necessario: numerose DDIs Epatite B: 6. Aderenza Facile aderenza ma attenzione alla compliance Epatite C: Gestione effetti collaterali, DDIs Counseling, team interdisciplinare (medici, infermieri, consulenti e associazioni pazienti)
