OBESITÀ
L obesità è una controindicazione all immissione in lista per trapianto renale?
We suggest that obese patients (BMI >30 kg/m2) present technical difficulties and are at increased risk of peri-operative complications. They should be screened rigorously for cardiovascular disease and each case considered individually. Although obesity is not an absolute contra-indication to transplantation, individuals with a BMI >40 kg/m2 are less likely to benefit.
Recommendations (B)Appropriately supervised weight loss therapy, combining low calorie diet, increased physical activity, and behavior therapy, is recommended for obese renal transplant candidates with a goal weight BMI < 30 kg/m 2 prior to renal transplantation. (C) There are few data to suggest which if any obese patients should be denied transplantation based on obesity per se.

Conclusioni
Rispetto alla dialisi, il trapianto renale comporta vantaggi in termini di sopravvivenza nei pazienti affetti da obesità, diabete mellito e sindrome metabolica Tutte e tre le condizioni rappresentano tuttavia importanti fattori di rischio per la sopravvivenza sia dell organo che del paziente
Compito del nefrologo è personalizzare la terapia immunodepressiva in base al profilo di rischio del paziente, soprattutto per quanto riguarda il diabete mellito, bilanciando il rischio immunologico con quello cardiovascolare Il coinvolgimento del paziente a condurre adeguati stili di vita (dieta, attività fisica), dovrebbe essere mandatorio, considerato il dono ricevuto e l inizio di una nuova vita.
