Carcinoma ovarico: conclusioni chemioterapia neo adiuvante vs chirurgia primaria La chemioterapia neo adiuvante è solo una opzione in pazienti giudicate non operabili (PS, comorbidità..) o tecnicamente non resecabili La chirurgia citoriduttiva primaria resta lo standard Necessaria una chirurgia citoriduttiva di alta qualità la chemioterapia neoadiuvante non è un rimedio per l assenza di una chirurgia di qualita Ma se proprio siete costretti ad un trattamento neoadiuvante

Bevacizumab si o no? Dal punto di vista regolatorio Le autorità regolatorie europee hanno approvato l'uso di Avastin in combinazione con carboplatino e paclitaxel per il trattamento in prima linea del carcinoma ovarico epiteliale, carcinoma peritoneale primario o carcinoma alle tube di Falloppio (stadio FIGO IIIb, IIIC e IV) Nella scheda tecnica non viene specificata alcuna limitazione riguardo a NACT o IDS

Conclusioni (se proprio siete costretti a somministrare un trattamento neoadiuvante ): 1. Tre, massimo 4 cicli e poi avviare la Paziente all intervento di citoriduzione di seconda istanza. 2. Il trattamento con carboplatino e taxolo rimane quello standard (si può aggiungere il bevacizumab ma sospendendolo almeno 28 giorni prima della chirurgia). 3. Studi in corso stanno valutando l attività e la safety dell uso specifico di bevacizumab nel setting neoadiuvante. 4. Considerare l aggiunta del bevacizumab dopo 28 giorni dalla chirurgia e comunque dopo la completa cicatrizzazione delle eventuali ferite chirurgiche o sedi di rimozione dei drenaggi.
