Lo strumento viene vivisezionato (visti i trascorsi), ma tutto OK Troponina dopo 5 ore (al ritorno del soggetto che era intanto andato a casa):.8 ng/ml Eseguiti ECG da sforzo, Ecocardiogramma tutti completamente normali e nonostante tutto il soggetto continua a stare benissimo. Terzo controllo di Troponina dopo 8 ore:.68 ng/ ml Giuriamo sulla validità del risultato e allora: coronarografia!!
CONCLUSIONI Le ragioni che fanno della Troponina un esame da eseguire in emergenza sono molteplici e debbono essere condivise con tutto il personale sanitario per avere tempi di esecuzione più rapidi possibile L eventuale utilizzo di un POCT (auspicabile?) non deve prescindere da questo L utilizzo della Troponina, così come quello di altri marcatori, è in continuo divenire e dimostra una sempre maggiore importanza diagnostica e prognostica Non bisogna dimenticare tuttavia un sempre utile atteggiamento critico nei confronti di qualsiasi risultato
