TESTS PER TROMBOFILIA Quando?
New anticoagulants : a concise review Baumann L.M et al J. Trauma Acute Care Surg 2012 (modified) Rivaroxaban (Xarelto) Apixaban (Eliquis) Edoxaban (Lixiana) Edoxaban Pradaxa
Evolving use of new oral anticoagulants for treatment of venous thromboembolism Yeh. C.H et al. Blood 2014
Evolving use of new oral anticoagulants for treatment of venous thromboembolism (Yeh. C.H et al. Blood 2014) HR for recurrent VTE and VTE-related death HR for major bleeding or major clinical plus clinically relevant non major bleeding Extended VTE treatment: Dabigatran,Rivaroxaban and Apixaban are superior to placebo and are associated with low rates of major bleedings. Apixaban at 2.5 or 5 mg BID significantly reduced the risk of recurrent VTE

Risks of recurrence after stopping anticoagulant therapy Chest Guidelines and Expert Panel report (2016) 1. VTE provoked by surgery (a major transient risk factor): 3% recurrence at 5 years 2. VTE provoked by a non surgical transient risk factor (estrogen therapy, pregnancy leg injury flight of > 8 h): 15% recurrence at 5 years 3. Unprovoked (also termed idiophatic) VTE ; not meeting criteria for provoked by a transient risk factor or by cancer ( 30% recurrence at 5 years) 4. VTE associated with cancer (also termed cancer-associated thrombosis ): 15% annualized risk of recurrence The risk of recurrence after: A. An isolated distal DVT was half that after a proximal DVT B. A second unprovoked proximal DVT or PE was 50% higher (1.5 fold) than a first unprovoked event. C. Man have about a 75% higher (1.75 fold) risk of recurrence compared with woman D. Patients with Positive D-Dimer have about double the risk of recurrence

Chest Guidelines and Expert Panel report (2016) Treatment of acute PE with NOAC that does not require heparin therapy (rivaroxaban, Apixaban) facilitates treatment without hospital admission. Patients who satisfy all of the following criteria are suitable for treatment of acute PE out of hospital Clinically stable with good cardiopulmonary reserve No contraindications such as a recent bleeding severe renal or liver disease or severe thrombocytopenia (<70.000) Expected to be compliant with treatment The patients feels wellenough to be treated athome The presence of right ventricular dyfunction or increased cardiac biomarkers level should discourage treatment out of hospital
