SORVEGLIANZA ATTIVA È UNA MODALITÀ DI MONITORAGGIO CLINICO DEL TUMORE DELLA PROSTATA CHE HA L OBIETTIVO DI EVITARE UN TRATTAMENTO NON NECESSARIO IN UOMINI CON TUMORI POCO AGGRESSIVI SENZA CONDIZIONARNE LA PROGNOSI NELL ATTESA DI INTERVENIRE CON UN TRATTAMENTO CON INTENTO RADICALE SE LA MALATTIA DIMOSTRI CARATTERISTICHE DIVERSE DA QUELLE ALLA DIAGNOSI.
I criteri di inclusione per la Sorveglianza Attiva variano considerevolmente tra i diversi studi pubblicati; la maggior parte includono: - PSA; - STADIO CLINICO; - GLEASON SCORE; - NUMERO DI CORES POSITIVI; - E/O MASSIMA PERCENTUALE DI TUMORE.

The PRIAS Study: Criteri di Inclusione 1. Diagnosi istologica di adenocarcinoma della prostata 2. Pazienti idonei ai trattamenti radicali 3. PSA alla diagnosi 10 ng/ml 4. PSA density (PSAD)N< 0,2 5. Stadio Clinico T1c-T2a 6. Gleason score 3+3=6 7. Massimo 2 core bioptici positivi È possibile arruolare pazienti con più di 2 campioni bioptici positivi, (indipendentemente dalla percentuale di malattia presente nei campioni), che abbiano effettuato una Risonanza Magnetica multiparametrica ed eventuale biopsia mirata delle lesioni. In caso di saturation biopsy con oltre 20 prelievi, il numero di campioni positivi deve essere 15% (fino a un massimo di 3 in caso di biopsia con 20-26 prelievi e 4 in caso di biopsia con oltre 26 prelievi).
