ANTICOAGULAZIONE E FIBRINOLISI ENDOVENA L effettiva anticoagulazione (un INR> 1.7 o un PT >15 sec) è allo stato attuale una controindicazione assoluta alla fibrinolisi endovenosa!! JACC 2014; 64: 1128-39 Nei pazienti che assumono i NOAC INR, PT e aptt non sono parametri adeguati per misurare lo stato coagulativo ed il rischio emorragico di tali pz ma dovrebbero essere eseguiti test specifici e standardizzati: TT, ECT 0 HEMOCLOT* per Dabigatran; ANTI-XA* per Rivaroxaban o Apixaban * Se normali situazione simile a INR<1.7 nei pz con TAO Si deve inoltre valutare: TEMPO trascorso dall ultima dose, FUNZIONALITÀ RENALE del paziente assunzione di ALTRI FARMACI che possono interagire con P-gpo citocromo P450 3A4
ALTERNATIVE STRATEGIES reversing the effect of a NOAC before thrombolysis?? L'Agenzia Europea per i Medicinali (EMA) e l italiana AIFA (GU del 23-2-2016) hanno espresso parere favorevole all immissione in commercio di Praxbind (idarucizumab) come antidoto specifico per l'anticoagulante Pradaxa (dabigatran etexilato), quando è richiesta una rapida inversione dei suoi effetti. Idarucizumab è indicato per pazienti in trattamento con dabigatran etexilato che debbano sottoporsi a interventi chirurgici di emergenza o in caso di sanguinamento incontrollato.
EMORRAGIA CEREBRALE (ICH) E ANTICOAGULANTI La mortalità dei pazienti con ICH e TAO è alta (52-73 %) Rosand, J, Arch Intern Med 2004; 164:880. Il principale indicatore di outcome sfavorevole è rappresentato dall aumento di volume dell ematoma E NECESSARIA QUINDI UNA RAPIDA CORREZIONE DELL ANTICOAGULAZIONE! Nei pazienti in TAO possono essere usati concentrati protrombinici (PCC), plasma fresco e/o vitamina K. Nei pazienti in NAO la gestione dell emorragia in urgenza rappresenta tuttora una grande sfida.

CONCLUSIONI la fibrillazione atriale è la causa più comune di ictus ischemico cardioembolico e/o criptogenetico per la prevenzione secondaria dell ictus tutti i pazienti con fibrillazione atriale che hanno una storia di ictus o TIA dovrebbero essere trattati con anticoagulazione permanente in assenza di controindicazioni i NAO devono essere preferiti al Warfarin per il migliore profilo di sicurezza ed efficacia e per la maggiore maneggevolezza di questi farmaci anche nella fase acuta dell ctus ischemico e/o emorragico
